Personal Information Protection Policy

As part of its activities, Mon Vet Privé Mobile Veterinary Service processes Personal Information. It recognizes the importance of respecting privacy and protecting the Personal Information it holds and is committed to taking the necessary measures to comply with the principles and requirements regarding privacy and personal data protection, as required by applicable laws and regulations.

Collection

The Service only collects the Personal Information necessary to carry out its activities. This information may include, but is not limited to:

  • Your contact details (name, address, phone number(s), email address(es))
  • Information about your pet (name, species, breed, sex, age, weight, medical history)
  • Appointment details, health conditions, and medical treatments provided to your pet
  • Payment information for services you have received

Use

The Service uses the Personal Information you provide to us for, among others, the following purposes:

  • Scheduling and managing appointments
  • Providing quality veterinary care tailored to your pet
  • Communicating with you about your pet’s care
  • Billing services and processing payments
  • Responding to your questions and concerns

Personal Information will not be sold to Third Parties or published online.

Disclosure

Information may also be shared when a Third Party is involved in your pet’s care. In such cases, the veterinarian and staff may need to share relevant Personal Information, consistent with the purpose of the record, with, among others:

  • External laboratories
  • Medical imaging interpretation services
  • Ultrasound services
  • When referring your pet to a specialist or transferring their file to another veterinary facility
  • For your insurance claims
  • When issuing an external prescription
  • Cremation services

We only disclose Personal Information with explicit consent (verbal or written), except where required by law, such as in the case of reporting a contagious disease or legal proceedings.

Retention

The Service takes all reasonable steps to ensure that the Personal Information it holds is up to date, accurate, and complete for the purposes for which it is collected or used.

The Service retains Personal Information for as long as required to carry out its activities. Under the regulations of the Ordre des médecins vétérinaires du Québec (OMVQ), a veterinarian must keep each record for at least 5 years from the date of the last service provided.

Destruction and Anonymization

Once the purposes for which the Personal Information was collected have been fulfilled, this information may be securely destroyed or anonymized.

Rights of Individuals

Any individual whose Personal Information is held by the Service has, among others, the following rights:

  • The right to access the Personal Information the Service holds about them and to obtain copies in electronic or paper format
  • The right to request the correction of any incomplete or inaccurate Personal Information held about them

Protection of Personal Information

The Service implements appropriate security measures to ensure the confidentiality and integrity of your Personal Information. Veterinary records are created in the context of providing care, are protected by law and professional secrecy, and are stored on a secure online platform accessible only to veterinarians and their staff.

Any confidentiality incident is handled in accordance with the Service’s confidentiality incident management procedure.
